The day-night pattern of colonic contractility is not impaired in type 1 diabetes and distal symmetric polyneuropathy

چکیده

Colonic contractility normally shows circadian variability regulated by sleep and especially food intake. However, individuals with type 1 diabetes have a reduced or even absent gastrocolic response to meal, indicating that colonic may be affected the disease. We hypothesized distal symmetric polyneuropathy (DSPN) decreased motility (expressed as index) of colon increase in index from night morning compared healthy controls without DSPN. Cohorts 35 DSPN, 40 28 were included this post-hoc, cross-sectional analysis. investigated, using wireless capsule measures pH, temperature, pressure throughout gastrointestinal tract, whether controls, exhibit altered evening, night, morning. Max amplitude, mean peak contraction, calculated at afore-designated times. Motility tended higher DSPN evening (P = .064), but effect size was small (1.74%). There no difference between groups evening. Furthermore, there max contraction morning, night. As expected, overall increased all groups, generally peaked almost Type and/or did not impair any time point. Contractility unaffected

Impaired Distal Thermoregulation in Diabetes and Diabetic Polyneuropathy

OBJECTIVE To determine how thermoregulation of the feet is affected by diabetes and diabetic polyneuropathy in both wakefulness and sleep. RESEARCH DESIGN AND METHODS Normal subjects, diabetic subjects without neuropathy, diabetic subjects with small-fiber diabetic polyneuropathy, and those with advanced diabetic polyneuropathy were categorized based on neurological examination, nerve conduct...

Accuracy of the Neuropad Test for the Diagnosis of Distal Symmetric Polyneuropathy in Type 2 Diabetes

OBJECTIVE To estimate the accuracy of Neuropad for the diagnosis and staging of distal symmetric polyneuropathy (DPN) across different stages of neuropathy, using multiple-level likelihood ratios (LRs) to interpret the time necessary to complete the color change of the test. RESEARCH DESIGN AND METHODS We conducted a cross-sectional, cohort-type diagnostic accuracy study in 251 consecutive ad...

The assessment of clinical distal symmetric polyneuropathy in type 1 diabetes: a comparison of methodologies from the Pittsburgh Epidemiology of Diabetes Complications Cohort.

Distal symmetrical polyneuropathy (DSP) is the most common type of diabetic neuropathy, but often difficult to diagnose reliably. We evaluated the cross-sectional association between three point-of-care devices, Vibratron II, NC-stat(®), and Neurometer(®), and two clinical protocols, MNSI and monofilament, in identifying those with DSP, and/or amputation/ulcer/neuropathic pain (AUP), the two ou...

Skin Intrinsic Fluorescence Correlates With Autonomic and Distal Symmetrical Polyneuropathy in Individuals With Type 1 Diabetes

OBJECTIVE To determine whether skin intrinsic fluorescence (SIF) was associated with autonomic neuropathy and confirmed distal symmetrical polyneuropathy (CDSP) in 111 individuals with type 1 diabetes (mean age 49 years, mean diabetes duration 40 years). RESEARCH DESIGN AND METHODS SIF was measured using the SCOUT DM device. Autonomic neuropathy was defined as an electrocardiographic abnormal...
